Contact: Aaron Lohr
alohr@endo-society.org
240-482-1380
The Endocrine Society

Chevy Chase, MD — Intermittent exercise with and without low oxygen concentrations (or hypoxia) can improve insulin sensitivity in type 2 diabetics, however exercise while under hypoxic conditions provides greater improvements in glycemic control than intermittent exercise alone, according to a recent study accepted for publication in The Endocrine Society's Journal of Clinical Endocrinology & Metabolism (JCEM).

Exercise improves glycemic control in patients with type 2 diabetes. Previous studies have focused on continuous aerobic exercise with more recent work assessing resistance exercise. There are few studies that assess the effects of intermittent exercise on glucose tolerance and insulin sensitivity in type 2 diabetes.

“Current guidelines suggest that health benefits can be gained when patients with type 2 diabetes spend 30 minutes exercising each day, but published data has failed to show intermittent exercise to be effective,” said Richard Mackenzie, PhD, of the University of Westminster in London, U.K., and lead author of the study. “Here we have shown that intermittent exercise seems to improve the glucose profiles of type 2 diabetics with a greater positive effect when intermittent exercise is combined with mild hypoxia, similar to doing the exercise at altitudes of 2500 meters.”

In this study, eight men with type 2 diabetes completed three exercise trials: 60 minutes of continuous exercise in hypoxia, intermittent exercise in hypoxia and intermittent exercise in a normal oxygen environment. After each exercise trial, study participants returned to the laboratory for measurement of blood glucose and plasma insulin levels. Researchers found that both intermittent and continuous exercise with and without hypoxia consistently demonstrated improvements in insulin sensitivity.

“A combination of moderately reduced oxygen levels with exercise can significantly improve the body's ability to respond to insulin in type 2 diabetic patients over exercise alone,” said Dr. Mackenzie. “Our findings suggest the possible use of exercise with mild hypoxic for the treatment of type 2 diabetes.”

    ###

Other researchers working on the study include: N. Maxwell, G. Brickley and P. Watt of the University of Brighton, U.K.; P. Castle of the University of Bedfordshire, U.K.; and B. Elliott of the University of Westminster.

The article, “Intermittent Exercise with and without Hypoxia Improves Insulin Sensitivity in Individuals with Type 2 Diabetes,” appears in the April 2012 issue of JCEM.

Chevy Chase, MD — Heart failure is associated with a 30 percent increase in major fractures and also identifies a high-risk population that may benefit from increased screening and treatment for osteoporosis, according to a recent study accepted for publication in The Endocrine Society's Journal of Clinical Endocrinology & Metabolism (JCEM).

Osteoporosis and heart failure are common, chronic and costly conditions that share common etiologic factors like older age, post-menopausal status and diabetes. Previous studies have suggested that heart failure may predispose a patient to fractures not only because it increases incidence of falling, but because both heart failure itself and its medical treatments can lead to loss of bone mass.

“Our study demonstrates for the first time that heart failure and thinning of bones go hand in hand,” said Sumit Majumdar, MD, of the University of Alberta in Edmonton, Canada, and lead author of the study. “Understanding the mechanism between heart failure and osteoporosis might lead to new treatments for both conditions.”

In this study, researchers conducted a population cohort study consisting of 45,509 adults undergoing bone mineral density testing for the first time and followed them for up to ten years. Of the 45,509 adults included in the study, 1,841 had recent-onset heart failure. After adjusting for traditional osteoporosis risk factors, researchers found that heart failure was associated with a 30 percent increase in major fractures.

“Part of screening for osteoporosis should involve looking at chest x-rays of patients with heart failure,” said Majumdar. “Heart failure patients get a lot of x-rays and they often incidentally show many fractures of the spine that would automatically provide an indication of severe osteoporosis and need for treatment.”

    ###

Other researchers working on the study include: Justin Ezekowitz of the University of Alberta; Lisa Lix of the University of Saskatchewan in Saskatoon, Canada; and William Leslie of the University of Manitoba in Winnipeg, Canada.

The article, “Heart Failure is a Clinically and Densitometrically Independent Risk Factor for Osteoporotic Fractures,” appears in the April 2012 issue of JCEM.

The authors say that sugar is contributing to the global obesity rates, which account for 35 million deaths a year world-wide from health problems, such as heart disease, diabetes, and cancer.

Currently, 75% of health care money is spent on diseases related to obesity and the problems that result from these diseases. Obesity problems and diseases that come from obesity are now more prevalent in the world than infectious diseases.

The team of scientists of endocrinology, sociology, and public health from UCSF began to look at the vast amount of evidence linking sugar to health problems.

The researchers say sugar is not just “empty calories”, rather, it has the ability to raise blood pressure, change metabolism, increase weight, and mix the signals of hormones, as well as causing damage to the liver, and is rapidly increasing health problems in the world. It can be noted that these problems are very similar to those caused by too much alcohol consumption.

The intake of sugar in the last 50 years has tripled, and is now thought to be the number 1 cause of the obesity problem spreading all over the world. However, Lustig, Schmidt, and Brindis said that 40 percent of the people with diabetes, heart disease, and cancer are not obese. Therefore, it is proven that sugar does much more than make people gain weight.

Fatherhood can be great, but only when you're ready.

A study published today in the journal of Reproductive Biology and Endocrinology offers new hope for men who would like to delay becoming fathers.

Basically, doctors would heat up the man’s testis with therapeutic ultrasound to the point that he would no longer produce sperm. It’s a completely painless, non-surgical treatment that takes just two 15-minute sessions and would ideally offer months of protection from accidentally getting a woman pregnant.

The downside is that for now, at least, it’s still a theoretical treatment that hasn’t been approved for use in humans.

Researchers at the University of North Carolina have confirmed they were able to perform the treatment on rats, which provided effective sterilization that appeared to be temporary.

Researchers at the University of California-Davis have tried to replicate the process on monkeys, but with limited results.

Still, some scientists are hopeful the technique can one day be adapted to work on human men.

“Sperm production is very robust; this ensures the survival of a species. It’s really difficult to find a way to turn off the production of sperm, but ultrasound seems to do the trick,” study author Dr. James Tsuruta, assistant professor of pediatrics, said in a press release. “There is something special about heating with ultrasound—it caused 10-times lower sperm counts than just applying heat.”

“It’s a nice idea, but a lot more work is needed,” Dr Allan Pacey, at the University of Sheffield told the BBC. He added that while sperm would likely recover and begin production again it would need to be proven that the sperm is not permanently damaged.

Still isn’t it nice to think there are scientists out there who are trying to help men take more control over their reproductive lives?

    January 30, 2012

Is there a simple, inexpensive, reversible male contraceptive coming to a clinic near you soon? A quick visit to a doctor’s office may soon be all that is needed for males to do their part for contraception.

The journal Reproductive Biology and Endocrinology details a study using commercially available therapeutic ultrasound equipment to reduce sperm counts of male rats to levels which would result in infertility in humans.

A team led by the University of North Carolina’s Dr. James Tsuruta found that by rotating high frequency (3MHz) ultrasound around the testes using two sessions consisting of 15 minutes ultrasound two days apart, they were able to cause uniform depletion of germ cells throughout the testes.

With saline used to provide conduction between the ultrasound transducer and skin and the testes warmed to 99º Fahrenheit, a Sperm Count Index of zero (3 million motile sperm per cauda epididymis) was achieved.

Tsuruta explains, “Our non-invasive ultrasound treatment reduced sperm reserves in rats far below levels normally seen in fertile men. Further studies are required to determine how long the contraceptive effect lasts and if it is safe to use multiple times. The perfect male contraceptive would be cheap, reliable, reversible, long-acting and have few side effects.”

A first attempt to use ultrasound as a male contraceptive was reported 40 years ago, reports Daily Mail UK’s Clair Bates, Several prostate cancer patients who were due to have their testicles removed underwent the treatment, which resulted in a “dramatic loss of germ cells”.

“These men reported that the procedure was pain-free, only creating a gentle feeling of warmth,” said the authors of the new study.

The idea however was not pursued at the time and the equipment used is now outdated and unavailable. The new research was intended as a starting point to see if modern ultrasound devices used for physical therapy can be used as a male contraceptive.

Dr. Allan Pacey, senior lecturer in andrology at the University of Sheffield, tells BBC News Health, “It’s a nice idea, but a lot more work is needed.”

He said that it was likely that there would be recovery of sperm production, but the “sperm might be damaged and any baby might be damaged”, when sperm production resumed. “The last thing we want is a lingering damage to sperm,” he said.
